Your first dental visit promises to be a pleasant experience.

To save time during your initial visit, please print and complete the New Patient Form and bring it to your scheduled appointment.  This information is required to ensure you receive the best dental care.  If you cannot complete these forms before your appointment, please arrive 15 minutes early to complete the form.

Making sound decisions about your dental care and oral health is an easy thing to do with the right preparation beforehand:

1. Make a list of questions to ask our office, so you don’t forget anything on the day of your appointment. This includes any concerns you have, or oral problems you’ve been experiencing.

2. Fill out the New Patient Form at home and bring it with you or arrive 15 minutes before your appointment to complete this at our office.

3. Call 317.883.3300 to schedule your appointment or complete our Request an Appointment form.

4. If you have dental insurance, remember to bring your insurance card with you.

We accept almost ALL dental insurances including Hoosier Healthwise/Medicaid. Call our office to confirm that we are preferred providers in your network.
